I was unable to get my pdf reader to function
properly, so I asked Doug for his entry in text along with the individual images.
That resulted in this follow-up email:

Here is the text for the instructions, and the important images I used in my
documentation. These images are size that were used in the publication. Larger size
pictures are available if you would like them.
Did you try removing Acrobat Reader and installing it again?
Let me know if you need anything else.

As a result, I have included Doug's original pdf
file but have also included the individual photographs just incase anyone else has
problems with the file.
